Of the 345 discharged persons served between 12/1/09 through 12/31/10, a total of 333 (97%) agreed to be contacted after discharge.  Of the 333 who provided contact information, 120 were successfully contacted based on the information provided on the Informed Consent for Follow-up Survey. representing a 36% response.  Surveys were conducted by telephone (17%), by staff that have contact with discharged persons served in meetings (66%), during Competency Restoration class (09%), and during Aftercare groups (08%).  Responses have been separated into two categories, 3-6 months and 7-12 months post discharge.

POST DISCHARGE 3-6 MONTHS

Of the 153 persons served identified in the 3-6 month post-discharge category, 59 were successfully contacted representing a 39% response.  The following recaps these responses.

Of the 59 contacted, 51 had no response or responded with praise.  Individuals not self-sufficient were offered contact information to Work Force One and/or Hispanic Unity.  Those indicating they did not have a support system were offered contact information for local meetings.  Other responses received were:

  3. I lost my cell phone
  4. Pay my car payments
  5. Help me get my SSI, my lawyer is no help
  6. Let me take the competency test so I can get on with my life
  7. Help me find a job
  8. Work 

POST DISCHARGE 7-12 MONTHS

Of the 180 persons served identified in the 7-12 month post-discharge category, 61 were successfully contacted representing a 34% response.  The following recaps these responses.

Of the 61 contacted, 55 had no response or responded with praise.  Individuals not self-sufficient were offered contact information to Work Force One and/or Hispanic Unity.  Those indicating they did not have a support system were offered contact information for local meetings.   Other responses received were:

  2. Help me get my memory back.  My meds help it not get worse
  3. Help me pass competency test
  4. Let me face my charges
  5. Let me face my charges
  6. I need a job

 

SUMMARY

Of the 345 discharged persons served from 12/1/09 through 12/31/10, a total of 333 (97%) agreed to be contacted after discharge.  This is a significant improvement over previous surveys.  In an effort to improve our percentage of clients agreeing to post discharge contact, we began having the consent to contact, which is completed during the admission process, revisited with the client 30 days prior to discharge.  Adding this additional step increased our consent for post-discharge contact approximately 27%.

Of the 333 who provided contact information, 120 were successfully contacted based on the information provided on the Informed Consent for Follow-up Survey representing a 36% response.  A total of 213 (64%) were unsuccessfully contacted having disconnected telephones, wrong numbers, or did not provide contact numbers.

Overall, of the successful contacts, 91% of persons served have remained drug free; 65% are self-sufficient; and 88% have a support system for their recovery. 

It was identified during the survey that 09% relapsed; 35% were not self-sufficient; and 12% did not have a support system for their recovery.

The outcome objective established to measure the success of persons served post-discharge is 65%.  We have significantly exceeded this objective.
